Terms and Conditions:

I understand when freezing my membership:

1. There is a $4 freeze fee each month my membership is frozen. And, that all freeze fee's collected each month are added to my "in store account".

2. There will be a $5 NSF added if Hawaiian TanFastic is unable to process my freeze fee.

3. That months that are frozen do not count towards active months and will not count if you chose to cancel your membership later.

4. That I can unfreeze early (pro-rated payment if unfrozen before next due date).

5. That all freeze fee's paid (accululated and saved in "in store acccount") can be used when my membership is reactivated. Freeze $$ can be used towards tanning product, upgrades, sunless sessions, red light therapy sessions, etc...

6. We, @ Hawaiian TanFastic, prefer to have 7 days advance notice to be able to freeze membership before your next due date. Every effort will be made to freeze your membership if your request is within 7 days of your next due date, but you may be charged your regular monthly payment.

7. If you need to freeze longer, you will need to complete another freeze form. Freeze forms can be completed online @ our website http://www.hawaiiantanfastic.com/ or either of the salons.
