Welcome to the Online Order form for the Choral Parents Association (CPA). This form will allow you to make one convenient, secure payment for your annual membership, wardrobe donation, Choir sweatshirts, program ads, pre-purchased concert tickets and more.
Please note that there are TWO steps:
Complete the online form and press CONTINUE. You will see a summary of your order. If correct, please proceed to payment. If you need to correct the order, click back and make your adjustments.
When you have finished reviewing your order, click the "Continue to PayPal" link at the bottom of the order summary. You will be directed to PayPal. You do not need a PayPal account, you may pay via credit/debit card.
You will receive an email from "Formsite" with a summary of your order after your payment has been confirmed. This may take a few minutes to process. If you do not receive this email, it means there was an issue with your payment, so please double check you completed it on PayPal. The subject line of this email will read: CPA Fall 2017 Online Order Form Result (with a number).
You will also receive an email from email@example.com confirming your successful payment, called Receipt for Your Payment to LCHS Choral Parents Association in the subject line.
Please keep copies of both confirmation emails. Please check your spam/junk folders for these emails if you do not see them in your in-box.
If you have more than three students in the Choir program, please complete an additional form as needed. Families with one student may simply skip the sections for Student #2 and #3.
