Online Order Form 2018-2019

Welcome to the Online Order form for the Choral Parents Association (CPA).  This form will allow you to make one convenient, secure payment for your annual membership, wardrobe donation, Choir sweatshirts, program ads, pre-purchased concert tickets and more.  
 
Please note that there are TWO steps:
 
Complete the online form and press CONTINUE.  You will see a summary of your order.   If correct, please proceed to payment.   If you need to correct the order, click back and make your adjustments. 
 
When you have finished reviewing your order, click the "Continue to PayPal" link at the bottom of the order summary.   You will be directed to PayPal.   You do not need a PayPal account, you may pay via credit/debit card. 
 
You will receive an email from "Formsite" with a summary of your order after your payment has been confirmed.  This may take a few minutes to process.  If you do not receive this email, it means there was an issue with your payment, so please double check you completed it on PayPal.  The subject line of this email will read: CPA Fall 2018 Online Order Form Result (with a number). 
 
You will also receive an email from service@paypal.com confirming your successful payment. The subject line of this email will read: Receipt for Your Payment to LCHS Choral Parents Association.
 
Please keep copies of both confirmation emails.  Please check your spam/junk folders for these emails if you do not see  them in your in-box.

Family Information

Choral Parents Association Membership

Select your preferred membership level below. All levels will receive recognition in our Concert Program.  Ticket sales from the Fall Pops Concert are designated for the Choir scholarship fund and are not included in the Membership program; you may purchase them below in the Concert Ticket Pre-Sale section.  Please note that businesses cannot be CPA Members.   We invite businesses to support the program with the purchase of a Program Ad, which is available below.  Membership donations are tax deductible to the extent allowed by law.  CPA Taxpayer ID #95-4760303

Platinum Level Membership:  $1000
Includes six adult tickets to the Fall, Holiday, Spring and Spring Pops concerts and choice of DVD or Digital Download recording for each of the three all-Choir concerts and the Spring Pops concert.  Note:  adult tickets may be used for students.

Gold Level Membership:  $500
Includes four adult tickets to the Fall, Holiday, Spring and Spring Pops concerts and choice of DVD or Digital Download of the Holiday Concert.  Note:  adult tickets may be used for students.

Silver Level Membership:  $250
Includes two adult tickets to each of the Fall, Holiday, Spring and Spring Pops concert.   Note:  adult tickets may be used for students.

Bronze Level Membership:  $100
Includes program recognition.

Please note:  Membership received after September 25, 2018, the Fall Concert deadline, will be acknowledged in the next available Concert Program.

Pre-Purchase Concert Tickets

Pre-purchasing is a great option if you have not elected to become a CPA member at a level that includes concert tickets, or if you need more tickets than your membership level grants, or if you wish to make your purchase now for the Fall Pops Scholarship Concert (this concert is NOT included in any of the membership packages).

No more fumbling with cash or checks at the door, simply pre-purchase tickets now and your name will be on our list. Unexpected guest? No problem, you can still purchase individual tickets at the door.

Ticket Prices:

Adults: $10 per ticket

Students: $5 per ticket (includes 7/8, 9-12 and college students).

Elementary students (K-6th) are free, no ticket purchase is required.

Tickets are non-transferable. Tickets may be used by family and guests of purchaser only. Guests must check in with purchaser.

Concert Program Business & Tribute Ads

Please consider the purchase of a business or student tribute ad in our Concert Programs.  Your ad will appear in up to 3 Concert Programs (Fall, Holiday & Spring) if ad materials are delivered by due dates listed below.

A portion of the proceeds from the sale of business or tribute ads will be applied to your student's tour fund.  Students are encouraged to sell ads to local businesses if they are eager to contribute to their tour expense fund. A separate online form just Ad Sales is available on the lchschoral.org website for students to access anytime throughout the year, so it it not necessary to return to this form.

All ads will appear in black/white/grayscale (no color) in sizes as noted below.  Sizes are in inches. 

Business Card Ad (3 1/2w x 2h) - $80

Quarter-page Ad (4w x 5h) - $125

Half-page Ad (8 1/2w x 5 1/2h) - $185

Full-page Ad (8 1/2w x 11h) - $300

Material Details and Deadline
 
Please email a digital file of your final black & white advertisement in (.pdf or .jpg) format to Carter Ransom ransom.cs@gmail.com.  Please inclulde "LCHS CPA Concert Program Ad Sales" in the subject line.
 

Deadline to have an ad included in our Fall Debut Concert Program is September 25, 2018.  Materials that are received after this deadline (but no later than Dec 3, 2018) will be held for insertion in the Holiday and Spring Programs.

 

Your ad must be in a finished format and must meet the size specifications noted above. CPA is unable to design ads on your behalf.  You must submit digital files of your ad layout, complete with any desired images, logos, and text, or you must have hard copy (such as a business card) that does not need editing and is ready to be scanned.  Hard copy ads may be mailed to Carter Ransom, 1994 Hilldale Dr. La Canada, CA 91011.

Annual Wardrobe Donation for Student #1.

Payment of the annual wardrobe rental fee for each student is requested annually.  Your donation helps offset repair and replacement expenses for the dresses and tuxedos. 

Note:  if your student is in Men's Ensemble and another Choir, only ONE wardrobe donation is requested.
Choir Sweatshirt for Student #1.

Each Choir has a unique sweatshirt, designed by one of the students.   Students wear them during field trips, tours and other events.  While not mandatory, it is strongly suggested that each student have a Choir sweatshirt.   This is the only time they will be offered.

If your student is in more than one Choir,  you may select a sweatshirt for each Choir.

Please note:   sizes for the sweatshirts will be collected by the students in class. *
Jewelry Purchase for Student #1.    DOES NOT APPLY TO 7/8 Students

If your daughter is new to a High School Choir (Women’s Ensemble, Concert Choir or Chamber Singers) or needs replacement jewelry, please select from the following:

Concert Choir and Chamber Singers:  please select either Pierced OR Clip-On Earrings option. 

Select Does Not Apply if your student is NOT in any of these Choirs or if she already has the correct jewelry.
