2018-2019 Concert Program Ad Online Order Form

Welcome to the LCHS Choral Parents Association (CPA) Concert Program Ad Online Order form.  This form will allow you to make one convenient, secure payment for your Program Ad and/or donation to our program.

Please note:  this online form is primarily for the convenience of businesses.   Choir families should use the General Online Form, which will be available at Back to School.   The General form also has option to purchase a program ad (business and/or student tribute).   However,  choir families who prefer to use this form for just a business ad may do so. 
 
Please note that there are TWO steps:
 
Complete the online form and press CONTINUE.  You will see a summary of your order.   If correct, please proceed to payment.   If you need to correct the order, click back and make your adjustments. 
 
When you have finished reviewing your order, click the "Continue to PayPal" link at the bottom of the order summary.   You will be directed to PayPal.   You do not need a PayPal account, you may pay via credit/debit card. 
 
You will receive an email from "Formsite" with a summary of your order after your payment has been confirmed.  This may take a few minutes to process.  If you do not receive this email, it means there was an issue with your payment, so please make sure that you have completed the PayPal payment process accurately.    The subject line of this email will read: "CPA Ad Sales 2018-2019  Online Order Form Result" (with a number). 
 
You will also receive an email from service@paypal.com  confirming your successful payment.  The subject line will read "Receipt for Your Payment to LCHS Choral Parents Association."
 
Please keep copies of both confirmation emails.  Please check your spam/junk folders for these emails if you do not see  them in your in-box.

Concert Program Business Ads

Thank you for purchasing a business ad. Your ad will appear in the Fall Debut Concert Program, the Holiday Concert Program, the Spring Concert Program, and the Spring Pops Concert Program.

The deadline to submit ad materials for the Fall Debut Concert Program is September 25, 2018. If your ad materials are received after the deadline, then your ad will be printed in the next program.



All ads will appear in black/white/grayscale (no color) in sizes as noted below.  Sizes are in inches. 

Businesss Card Ad (3 1/2w x 2h) - $80

Quarter-page Ad (4w x 5h) - $125

Half-page Ad (8 1/2w x 5 1/2h) - $185

Full-page Ad (8 1/2w x 11h) - $300

Material Details and Deadline
 
Your confirmation email will contain a link to a shared drive to which you can upload your ad materials. Please ensure you keep the email so that you can upload your ad materials.
 
Or, you can email a digital file of your final black & white advertisement in (.pdf or .jpg) format to Carter Ransom ransom.cs@gmail.com.   Please inclulde LCHS CPA Concert Program Ad Sales in the subject line.
 
Deadline to have an ad included in our Fall Debut concert is September 25, 2018.    Materials that are received after this deadline will be held for insertion in the next available program.  
 
Your ad must be in a finished format and must meet the size specifications noted above. CPA is unable to design ads on your behalf.   You must submit digital files of your ad layout, complete with any desired images, logos, and text, or you must have hard copy (such as a business card) that does not need editing and is ready to be scanned.  Hard copy ads may be mailed to Carter Ransom, 1994 Hilldale Dr. La Canada, CA 91011.

General Donations to LCHS Choral Program

Our students flourish when the community supports their interests.  We are grateful for your memberhip donation and you will receive name recognition in our program.   

Levels include:  
Platinum:  $1000
Gold:  $500
Silver:  $250
Bronze:  $100
or, any amount is gratefully accepted.
