Chamber Singers Fall Retreat September 28 - 30, 2018

Welcome to the Chamber Singers Fall Retreat Payment Form for the LCHS Choral Parents Association (CPA). 
 
Please note that there are TWO steps:
 
Complete the online form and press CONTINUE.  You will see a summary of your order.   If correct, please proceed to payment.   If you need to correct the order, click back and make your adjustments. 
 
When you have finished reviewing your order, click the "Continue to PayPal" link at the bottom of the order summary.   You will be directed to PayPal.   You do not need a PayPal account, you may pay via credit/debit card. 
 
You will receive an email from "Formsite" with a summary of your order after your payment has been confirmed.  This may take a few minutes to process.  If you do not receive this email, it means there was an issue with your payment, so please double check you completed it on PayPal.  The subject line of this email will read: CPA 2018 Fall Chamber Retreat Online Payment Form Result (with a number). 
 
You will also receive an email from service@paypal.com  confirming your successful payment. The subject line of this email will read: Receipt for Your Payment to LCHS Choral Parents Association.
 
Please keep copies of both confirmation emails.  Please check your spam/junk folders for these emails if you do not see  them in your in-box. 

Chamber Singers Retreat Information

Retreat Dates:  Friday, September 28 - Sunday, September 30, 2018.
 
Cost per Student:  $200.00
 
Online Confirmation and Payment deadline is September 14, 2018.
 
It is important that a parent or legal guardian complete the online registration form for each student.
