Welcome to the 2023-24 Family Online Order Form for the Choral Parents Association (CPA)!
This form will allow you to make one convenient, secure payment for:
1) sponsorships or general donations
2) annual CPA memberships
3) family tribute ads
4) wardrobe donations
5) retreat payments for those choirs going on retreats
In addition, you will be able to:
1) submit your family and student data
2) sign up if you're interested in volunteering with CPA
3) opt in or out for consent to have your student photographed
