Pre-casting rehearsal, auditions and rehearsals will be held at 13921 7th Street, Dade City, FL 33525

Rehearsals: Tuesday & Thursday from 6:00 - 8:30 p.m. and Saturday 9 a.m. - 12:00 p.m. beginning Tuesday, August 16. Please expect extended rehearsals, especially closer to show time. Extended rehearsals will be announced at a later date.

Tech Week & Performances: Tech Week is October 17 -19 and the show is scheduled for October 20 - 22 at the Instructional Performing Arts Center at Cypress Creek High School. No conflicts will be accepted for tech week or show dates.

Cast members ages 14 and up are allowed to sign themselves in and out of rehearsal. Cast members age 13 and under must be signed in/out by a parent or sibling (with permission.) Cast members may only sign themselves in and out at the designated beginning and end time of rehearsal unless specified with a signed parental note. I understand that once my child has signed themselves/their siblings out of rehearsal, Arts in Motion is no longer responsible for my children. *

2022-2023 Release of Liability & Permission to Secure Treatment: I recognize and acknowledge that there are certain risks of physical injury to participants in the above program(s) and I agree to assume the full risk of any injuries, damages or loss regardless of severity which I or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with such program(s). I agree to waive and relinquish all claims I or my minor child/ward may have against Arts in Motion (AIM) and its volunteers as a result of participation in the program. I recognize that AIM is a non-profit, volunteer organization and serves children from grades Kindergarten through 12th grade. As such, by virtue of our participation and membership in the organization, I agree to abide by the decisions made for the best interests of the organization and understand that while special requests may be made, they may not always be accommodated. Should a dispute or concern arise, I agree to bring it to the attention of the AIM Board first and attempt to work toward a mutual resolution. Should a mutual resolution not be reached, I hereby waive and relinquish any and all claims I or my minor child/ward may have against Arts in Motion and its volunteers and agree that I will not publicly disparage the organization and/or its members, seek damages of any sort or file any suit or claim against Arts in Motion and/or its Board and/or volunteers. I do hereby fully release and discharge AIM and its volunteers from any and all claims from injury, damage or loss with the activities of the program(s). I further agree to indemnify and hold harmless and defend AIM and its volunteers from any and all claims resulting from injuries, damages, and losses sustained by me or my minor child arising out of, connected with, or in any way associated with the activities of the program(s). 2021-2022 Participation Agreement and Discipline Guidelines

To create a harmonious, safe and productive environment, the following guidelines are to be accepted by cast members and parents. The success and enjoyment of our production experience largely depends upon mutual respect, cooperation, and responsibility.

Rules for All Arts in Motion (AIM) Productions/Workshops/Camps/Events
1. Participants must arrive on-time to all rehearsals unless they have listed a prior conflict on their registration form or notified the VP of Production or other designated person of a prior conflict. Unexcused tardiness on more than three occasions can result in the removal of your role.
2. Participants may be removed from their role in the production if they miss rehearsals without listing a prior conflict on their registration/audition forms. Conflicts that arise after the rehearsals begin must be given to the VP of Production and approved by the Stage Manager or other designated person within 48 hours of schedule distribution.
3. Participants will not possess or bring any form of weapon to any AIM function. This may be grounds for expulsion from the show or the organization.
4. Acts of Physical, Verbal or Emotional violence against a fellow cast member, board member or production team member may be grounds for expulsion from the show or the organization.
5. Participants will treat other cast members and staff with courtesy and respect.
6. Participants will not bring, use, or be under the influence of drugs, alcohol or tobacco products including tobacco alternative products such as e-cigarettes, vape pens, etc.
7. Participants will keep electronic devices, including cell phones, stored during instructional times. Electronics are NOT ALLOWED at any rehearsal.
8. Participants will not touch items belonging to another cast member or to Arts in Motion.
9. A responsible adult who drives cast members age 13 and under must walk into rehearsal and sign them in and out of rehearsals.
10. Public displays of affection in a romantic nature (PDA) are not acceptable.
11. All participants will dress appropriately and wear appropriate dance or athletic shoes for all rehearsals. NO OPEN-TOED or OPEN-BACK shoes allowed at any AIM event.
12. Participants are not permitted in areas not monitored by Arts in Motion volunteers.
13. All facilities and items belonging to AIM will be kept clean and treated with care and respect.
14. Participants will not be involved in any activity or promotion of any illegal activity at any time or location.
15. All AIM members agree to promote the organization in a positive way and agree that they will not publicly disparage or make negative comments about the AIM organization, another member of AIM, the AIM Board or the Production Team. This includes verbal comments as well as posting on any social media or internet forum. All issues should be brought to the attention of the appropriate AIM Board representative and dealt with in a professional manner.

The AIM board reserves the right to remove any participant from the program for violating these rules or for other reasons that we deem necessary.

The Arts in Motion (AIM) Board of Directors has set the following discipline guidelines in order to ensure that all AIM participants have the opportunity to participate in an arts education environment that is safe and productive. We want you to know that we will make sure that all children are being treated fairly and considerately and that the Production Team (PT) and the AIM Board are able to provide a positive learning environment for cast members and participants.

The AIM participant(s) who breaks one of the guidelines for disruptive behavior listed below will be given two chances by the PT to change his or her behavior. If that behavior (or a different but still disruptive behavior) continues, the cast member will be escorted respectfully to the designated AIM Board Member, usually the VP of Production. If the behavior(s) continues, parents will be called and expected to pick the cast member up immediately. Any participant that has been written up three times and/or picked up three times by their parent due to disruptive behavior during the course of a production or event will be dismissed from the production and no fees returned.

Please Note:
1. The AIM Board expects all members of the PT to be consistent in the determination of what is "disruptive behavior" based on the guidelines set up by the AIM Board and we expect the PT to treat each cast member equally and fairly.
2. AIM Board Member(s) will be involved in any decision for dismissal. Having a Board Member involved will add consistency since Board Members are more likely to know the cast members and parents since we work with them show to show. It is our role as a Board to protect each child's best interest and make sure that all cast members are treated fairly and with respect at every stage of a production or event. However, the Director has the final say on whether a participant(s) is dropped from a production or event due to consistently disruptive behavior as outlined above.
3. The AIM Board Member in charge will make a written comment using an Incident Report at the time each event occurs with an explanation of the disruptive behavior as well as the consequence given to ensure consistency and fairness to all cast members and to assist the cast member and their parents in amending behavior.

Photo/Video Release

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

Photographic, audio or video recordings may be used for, but is not limited to, the following purposes:

• promotional efforts
• educational presentations
• informational presentations
• on-line presentations and promotion
• educational videos

By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in public for promotion.

I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected as part my participation in Arts in Motion in the 2020 -2021 season.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.

Covid-19 Waiver

If you choose to participate in any AIM function, you must follow the guidelines in this form. Because the virus that causes COVID-19 can be spread to others by infected persons who have little or no symptoms, it is important to follow the practices specified in these protocols. If you cannot follow these guidelines, for your safety and the safety of those in attendance, you should remain at home.

Each day prior to attending any AIM function, all participants should self-screen for any of the following new or worsening signs or symptoms of possible COVID-19. If you have any of the following, please notify us and you will be excused from rehearsal:

• Cough
• Shortness of breath or difficulty breathing
• Chills
• Muscle Pain
• Headache
• Sore Throat
• Loss of taste or smell
• Diarrhea
• Feeling feverish or a temperature greater than or equal to 100.0 degrees Fahrenheit
• Known close contact within the last 14 days with a person who is confirmed to have COVID-19

The nature of theatre makes social distancing difficult, however, AIM is doing everything we can to mitigate the risk of COVID-19. Our mitigation plan includes:

• Limited persons in the building (when possible.)
• Hand sanitizer readily available.
• Volunteers available to sanitize high touch surfaces and shared equipment.
• A quarantine protocol should a person at AIM develop symptoms of COVID-19.

These precautions are important, but they do not fully remove the potential for exposure to COVID-19 or any other illness while in attendance.

The participant listed below, with my parent/guardian have chosen to participate in Arts in Motion activities during the 2022-2023 year. (July 2022-June 2023)

I understand that COVID-19 is highly contagious and governmental authorities have recommended measures such as social distancing, the wearing of masks, and limits on group sizes. These measures may or may not be adhered to during the duration of the activity.

I and my parent/guardian agree that participation in AIM activities is voluntary and have carefully considered the risks involved in participating. I hereby fully release and hold harmless, on behalf of myself and the participant(s), Arts in Motion Community Youth Theater Arts Education, Inc., fellow participants, or other persons/organizations associated with the activity.
