2023 Holiday For Kids' Sake SPONSOR SIGN-UP FORM

Sign-ups are ongoing over the next few months. Wish lists are collected from our children/families in late fall and you will be contacted mid November with all important information. Such as, directions for shopping, wish lists and instructions for packaging, delivery and drop off of items. Each child submits 3 wish list items, valued at $40 or less, and as a sponsor you are committing to purchasing those three items or similar.

Upon completion of this form, you will receive a confirmation email from dtaylor@bbbsli.org or mmichaels@bbbsli.org within 5 days. If you do not, please contact events@bbbsli.org.

ALL GIFTS NEED TO BE DROPPED OFF NO LATER THAN DECEMBER 1st TO ONE OF THE FOLLOWING OFFICE LOCATIONS:
