Certificates and Seals Order Form

PTDI standards require training organizations provide each graduate of a certified course with an official PTDI certificate and/or seal of attainment.  Certificates and seals have unique serial numbers.  Schools must track the serial numbers issued to each graduate (including voids) and report them each year in the Certification profile using the required PTDI Certificate Report form.  Find the guidelines and the reporting form under at: http://ptdi.org/CertProfile
 
PTDI certificates: may be issued by themselves or in addition to your own school certificate
PTDI seals: must be affixed to your own school certificate

Please allow a minimum of 15 days for order to be processed. For expedited orders an additional rush fee will be charged.  Certificates are normally shipped via FedEx.  If ordering less than 25 certificates or seals, a processing fee will added to your order. Contact PTDI for more information.
