Fall Skills Camp Registration November 9th

Basketball Skills Camp:

Saturday, November 9th

1:30-4:30PM

*Check in will start at 1:15PM

- Camp is open for 3rd-8th grade boys and girls

- Venmo is accepted to avoid processing fees (@HammerBasketball)

- Please send email to hammerbasketball@outlook.com to confirm payment

- The camp will focus on skill development, games, 3 on 3, and 4 on 4. The camp will be open to all players beginner and advanced.

- At 3:30PM the Beachside Boys Basketball team will have a team scrimmage. All parents, campers, and spectators are welcome to join.

- Location is at Beachside High School (200 Great Barracuda Way, St Johns FL, 32259)

Basketball Camp Injury & Liability Form

Please read carefully.

I hereby authorize the staff of Matt Clark and Hammer Basketball to act for me according to their best judgment in any emergency requiring medical attention. I hereby authorize and release the staff of Hammer Basketball (The Positive Hammer LLC) and Beachside High School (St. Johns County School District) of any and all liability for any injuries or illnesses that occurred while under their direction. The undersigned parent, guardian, or legal representative further acknowledges that he/she is authorized to enter this agreement on behalf of the child, child’s parents, personal representatives, assigns, heirs, and next of kin.

I further authorize any representative of this camp to obtain medical treatment for my child in the unlikely event of injury or illness during this program, and I agree to pay any expenses incurred for such treatment.

By signing this waiver, I also authorize the use of pictures of the participant(s) to be posted through the camp website or advertising media (social media) by Matt Clark and Hammer Basketball (The Positive Hammer LLC).
