In lieu of my credit card imprint, I hereby authorize JETT SET TRAVEL and/or their travel partners to charge my credit card as detailed below:

I authorize Jett Set Travel to charge the credit card or bank account indicated on this
invoice for the noted amount on today's date. I understand that my right to a refund in the event
of cancellation is governed by the cancellation and refund policy in Jett Set Travel's Terms
and Conditions of said Booking. I certify that I am an authorized user of this credit card, that I
agree to Jett Set Travel's Terms and Conditions of said Booking, and that I will not dispute
the payment with my credit card company so long as the transaction corresponds to the amount
and terms on this invoice.

By submitting this form, I acknowledge and accept the charges described herein and understand that should my travel plans change, and I did not purchase insurance, I may not be entitled to a full refund and that cancellation or change fees may apply.  Credits/Refunds are not guaranteed due to global events or acts of God.   If I purchased insurance, my cancellation penalties would be adjusted or waived, based on the terms of the policy purchased. 

I accept that Jett Set Travel, its travel suppliers, and its independent agent(s) are acting fully upon my direction and that I will not hold them responsible for acts beyond their immediate control as it relates to this charge and that I will make payment according to the terms of my agreement with the credit card company.  I will not hold Jett Set Travel accountable for any decisions made by the vendor in which my booking is made should I have to cancel, change or postpone my travel plans.

I will not hold Jett Set Travel responsible for any unforseen medical events or disaster that myself or any persons traveling with me experience.   I will not hold Jett Set Travel responsible for any unknown restrictions that keep myself or my travel party from traveling. 

It is the sole responsibility of the client to check the countries you wish to visit for safety precautions as each travel advisor can not speak to the accuracy of those reports.  All safety concerns must be considered by the client only.  Jett Set Travel is held harmless.

All international travel requires a CURRENT passport that is valid for 6 MONTHS AFTER TRAVEL

All NON U.S. CITIZENS traveling internationally are required to have a valid Permanent Resident Card AS WELL AS a valid passport. 

A state issued birth certificate with a government issued photo ID or Passport "Card" is not acceptable for air travel outside of the United States.

The name on your passport MUST MATCH the name on your travel documents exactly or you will NOT be eligible to travel. A copy of all travelers' passports should be submitted to Evan's World Travel PRIOR to picking up travel documents. 


Coronavirus COVID-19 Release, Assumption of Risk, Waiver of Liability & Indemnity Agreement

Jett Set Travel wants you to have a safe and truly wonderful vacation, so we recommend that you fully educate yourself on the destination and hotel specific changes that have been made in response to the COVID-19 pandemic, and plan accordingly before you depart on your vacation.

By signing this agreement, the traveler is fully aware of the current global Coronavirus COVID-19 virus outbreak, the current travel restrictions, and inherent risks involved in choosing to travel and purchase arrangements made by Jett Set Travel.  As the traveler, I understand that it is my responsibility to check the latest travel information regarding this virus outbreak and that I should have travel insurance to ensure I have coverage for all medical needs and trip cancellation, but understand that medical expenses/and or cancellation relating to the Coronavirus COVID-19 may not be covered.

Travel insurance generally only covers unforeseen events. Most insurers classified COVID-19 as a 'known event' in late January 2020. This means that if you booked before that, you may still be covered; if you booked after 31 January, it's unlikely you'll be covered for anything relating to COVID-19. Please note that most policies have a specific clause stating they do not cover epidemics and pandemics, especially when travel warnings are in place. Check with your insurance company for coverage details. A few supplier partners may offer Cancel-For-Any-Reason insurance or waiver options to protect your vacation investment and is highly recommended during the pandemic.

As the traveler, I confirm that I have adequate financial resources in case medical care is needed relating to Coronavirus COVID-19 and understand the inherent and potential health risks of travel during this time and that in some destinations, medical attention may be days away. I, the traveler, am aware of my home country's travel warnings, travel restrictions and rules and understand the risks, and is accepting of these knowing that Jett Set Travel cannot be held responsible for any travel restrictions, illness, cancellations, financial loss, quarantining rules or measures put in place at airports or destinations you are traveling through independently or as part of the vacation. I, the traveler, am aware that additional screening procedures and restrictions may take place at airports and in public areas. Immigration restrictions may be put in place before or during my travels.

It is my duty to check with my destination country's immigration department to confirm entry to the country and also my home country/final destination for their regulations. I, the traveler, am aware that it is my personal decision to travel and is doing so with full knowledge of current travel recommendations and travel restrictions with regards to the Coronavirus COVID-19 and takes full responsibility for his/her actions with regards to this. Some employers may require a 14-day quarantine when traveling abroad. It is recommended to check this with your employer before booking your trip.

COVID-19 TEST MAY BE REQUIRED TO ENTER YOUR DESTINATION. Failure to have the proper COVID-19 documentation may affect entry into your destination. It is the traveler’s responsibility to have all the necessary documents, tests, and vaccinations completed prior to departure. Improper required travel documentation is not covered by any Trip Protection Plans.

Since destination entry requirements vary widely and change frequently, at least 5 business days prior to departure, we recommend that all Travelers:

  • Check your destination’s official governmental health ministry website for the latest entry, testing, and in-destination self-isolation requirements and complete all pre-travel items and preparation; e.g.  
  • Check the CDC website for all healthy safety advice for travel advisories during the pandemic. 
  • On-site service adjustments may include but are not limited to, food services (room service, buffets), check-in & check-out time, daily linen change, turndown service, on-site amenities (pool, spa, gym), etc. 

Jett Set Travel customer needs to complete and submit their signature as a record that they have read and understood the above Coronavirus COVID-19 Release and Waiver form.
