If you are here, you should have completed the online registration through Phenix City Schools Enrollment, and now appropriate documentation may be required for the enrollment to be completed and class assignments released. This form should only be completed by the parent/guardian that is legally responsible for the student being enrolled with Phenix City Schools. Failure to provide appropriate information and documentation may cause an enrollment denial or delay. Returning Student Re-Identification Required Documentation for the 2020-2021 School Year

In our efforts to reduce contact and maintain the safety of Phenix City Schools staff, parents, and students, Reidentification for the 2020-2021 school year will be electronic. In order to to finalize your student's registration and receive their class assignment(s), your Powerschool Enrollment registration must be completed by using your student's assigned snapcode, and all documents required must then be submitted to this form. Please upload each piece of required documentation as requested. Once all uploads are reviewed and accepted, you will be contacted by school personnel regarding further information necessary and/or class assignments. Please complete one separate submission for each student returning to Phenix City Schools.
Required Documents for Re-Identification within Phenix City Schools
 
1. Updated Al Vaccination Record if current is expired
a. Notifications will be sent via email and phone message for those in need.
b. Students must have this in order to physically attend classes.

2. Proof of Residency (POR) required if address has changed.
a. Address must be in district, unless an approved Request for Transfer has been obtained from the Phenix City Board of Education.
b. Must have guardian name.
c. Must be within the last 60 days.

 

Required Documents for Enrollment within Phenix City Schools

1. Birth certificate- Guardian completing registration must match parent(s) on the birth certificate or appropriate legal documentation must be provided. Powers of Attorney nor notarized documents are accepted for proof of guardianship.
2. Social Security Card- Must match student name registering.
3. Parent Identification- Drivers' license, military identification, or passport are acceptable.
4. Al Vaccination Record (must have in order to physically attend classes)
5. Previous school proof of enrollment with grade level/promotion documentation (i.e. Withdrawal form, final report card, unofficial transcript, etc). 
6. Proof of Residency (POR)
a. Address must be in the district, unless an approved Request for Transfer has been obtained from the Phenix City Board of Education.
b. Address must pull up in Edulog- must have approved Request for Transfer if not in Edulog and out of district.

Transportation Information

Please fill out the information below as completely as possible.  Teachers will use this sheet to dismiss your student(s) at the end of the school day unless a change of transportation notice is received for your child.  During the first few weeks of school, transportation changes need to be made in writing. Bus transportation will only be provided to the address specified in PowerSchool registration.  Any changes to Bus Transportation must be made on or before July 15th to the LES office.  Any transportation changes after July 15th could result in your child not being transported by Phenix City schools until August 31st.  As always, if you live outside the PCS district, you are required to provide your own transportation.  Thank you for your understanding.
 
Please note:  Choosing After School does not guarantee a spot in Lakewood Elementary's LEAP Program.
All car riders must be picked up by 3:20 PM each day.  There will be NO school check-outs after 2:30 PM each day to allow for us to transition into dismissal procedures.  All parents/guardians MUST have a car rider sign for pick up each afternoon.  This sign should be placed in the front windshield of the car.  A temporary car rider tag will be emailed to you for the first day of school.  Please print and place this in the front windshield prior to pick up on August 17th.  A permanent car rider tag will be sent with the student on the first day of school.  

Medical Information

In the event of an emergency, or if school officials deem it necessary, your child will be taken by ambulance at the parent's expense to the nearest emergency facility, where the attending physician (or dentist) on duty may perform emergency treatment on your child.
If you have prescription medication that needs to be given to your child during the school day or a medical condition warranting a diet plan, please contact the school at 334-732-1173 to make an appointment during the week of August 10th-August 14th to bring the medication and appropriate form to the school. You will need to print the medication authorization form and/or the diet prescription form on the PCBOE website and bring it (along with the medication) to the school during your scheduled appointment time the week of August 10th-August 14th.  

Agreement

Your electronic signature at the conclusion of this form serves as your verification that all answers and provided documents within this form are valid and accurate. It also serves as your agreement to adhere to the PCBOE's Student Code of Conduct and enter into a partnership role with the school for your student's education.  You will receive an electronic copy of your submitted form to the email provided for your records as well.
