Parent Route Change Request Form

Prior to the beginning of school, transportation requests must be completed before July 14, 2024, to be honored. Any changes after this date will not be accommodated until the second week of school.
During the school year, all transportation change requests must be submitted by 11:30 pm (EST) the day prior to the requested date.  Your request is not completed until you receive approval or denial from the school office.  You must live within the Phenix City Schools' zone for transportation.
You must arrive at the stop 10 minutes before the scheduled pickup.
Route change requests are to be completed for valid change of residence or to the pre-approved after school programs. 

Request for afterschool programs may be approved pending available space on the bus.  It is the parents responsibility to ensure the program has space available prior to submitting any request for transportation.

For a change of residence, I have until July 14 to make any changes.  After that date, transportation may not be provided until 2 weeks after the start of school (August 21).  Any change of home address will need to include updated proof of residency.
