Waiver of Limited Liability, Terms & Conditions of Pretty Dollfaces INC. Membership
This Waiver of Limited Liability is valid for all trips/destinations with the Pretty Dollfaces INC. that you partake in from the day you purchase your membership until the final expiration date of membership.
Terms of the Waiver of Limited Liability
1. In signing the reverse of this form I (the member) RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Pretty Dollfaces, INC., its trustees, directors, agents or employees from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or any relating to any loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, while travelling to/in the international or domestic destination(s). I voluntary assume full responsibility for any risks of loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, while engaging in social activities or travelling to/in the international or domestic destination(s).
I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING THE MEMBERSHIP FORM ON THE REVERSE, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
Terms & Conditions
Pretty Dollfaces INC. (PDI) is a registered corporation (document number P160082000) which runs the Pretty Dollfaces as part of its program of activities. All trip/vacations (with the exception of most day-trips) are planned and organized by a dedicated in-house Travel Coordinator.
These are the standard booking conditions of Pretty Dollfaces INC.
1.Agreement
By becoming a member of Pretty Dollfaces INC. you agree to accept the full Terms & Conditions (below) and Waiver of Limited Liability of the Pretty Dollfaces Inc. available on the application form.
My signature below confirms the following:
______All members will be listed in the directory of services, however you may choose to redact some information. I.E. contact numbers, physical address.
______ I hereby authorize Pretty Dollfaces, INC(PDI) to publish photos of me without payment and my name or likeness for use in PDI’s website, newsletter, social media , video based marketing materials, as well as other Company publications.
2. Membership*
Membership is open to all mature females aged 21 or older. Proof of age must be provided showing a current state issued identification when joining. Membership can be obtained in person at the Pretty Dollfaces INC. Events or online on www.meetup.com.
3. Your application will be reviewed within 48 hours excluding weekends and holidays. It will be followed with a telephone call from the director of member services.
*Pretty Dollfaces INC. reserves the right to refuse or cancel membership at any time at its discretion. It also reserves the right to remove any violators of the code of conduct immediately from a social event or trip if it inhibits any and all participants from being able to enjoy a pleasurable sisterhood experience.
Permission to Use Photograph and Likeness
I grant to Pretty Dollfaces, Inc., its representatives and employees the
right to take photographs of me and my property in connection with
the above-identified subject. I authorize Pretty Dollfaces, Inc., its
assigns and transferees to copyright, use and publish the same in print
and/or electronically.
I agree that Pretty Dollfaces, Inc. may use such photographs of me
with or without my name and for any lawful purpose, including for
example such purposes as publicity, illustration, advertising, and Web
content.
