In order to be approved to discontinue sending in paper timesheets, all caregivers must read and sign this form to acknowledge that they understand the rules and requirements of using the Care Worker app to complete timesheets. Failure to follow these rules could result in termination of position. Repeated mistakes on the Care Worker app could result in continued requirement to complete and turn in paper timesheets.
