As an employee of Trillium Services, you are eligible to participate in the 401(k) retirement savings plan administered by Securian Retirement Plan Services. The plan offers a convenient way to invest in your future by allowing you to make pre-tax contributions via payroll deduction.
To ensure that you take advantage of this great benefit, you can be automatically enrolled in the pre-tax 401(k) retirement plan.
Please note the following important factors when participating in automatic enrollment
* If you DO NOT electronically sign and submit this form, you will be automatically enrolled in the 401(k)
plan.
* 1% pre-tax contribution to the plan is deducted each pay period beginning the first pay period after
30 days of employment.
* For rehired employees who have no previous investment elections on file, contributions will be invested in the 1% investment option. If you previously participated in the plan, contributions will be invested
according to your most recent investment elections.
If this form is not received by the Payroll Department by the 30th day after your hire date,
you will be automatically enrolled in the 401(k) plan, contributing 1% of your pay.
