Parental Acknowledgement of Use of Email Addresses 
By checking this box, I, as the parent/guardian of Student Member of TWHS Bass Fishing Team, consent to the team providing the email address(es) above to team sponsors for distributing product information and marketing material.  All BFT sponsor discounts and promotions will be communicated to members using the email address(es) provided. *
INSHORE SERIES PARTNER INFORMATION: Most anglers choose to fish with a partner but it is not required. Please indicate your partner status below. *
Student Physical

All student anglers are required to submit a CISD Pre-participation Physical to join the team. A physical must be on file with the team by September 1st to complete your registration and hold your spot on the team. A new, current physical must be taken and submitted each school year to participate.  A copy of the 2019-20 entire required physical packet to be completed and submitted can be found at: http://www.conroeisd.net/wp-content/uploads/2016/04/Pre-Participation-Form-2019-20.pdf
If you do not have a physical for 2019-20 on file with CISD for another activity, please print and submit this entire form and physical to the Bass Fishing Team.

Parental Acknowledgement of Use of Participant's Personal Vehicle
By checking this box, I, as the parent/guardian of Student Member of TWHS Bass Fishing Team, understand that the Conroe Independent School District (CISD) will not provide transportation to any in-District, off-campus school-approved facilities for the purpose of practicing or competition for the named activity (Bass Fishing). I understand that as a condition of participation in the activity of Bass Fishing, my child must obtain his or her own transportation to the in-District off-campus school-approved facility to practice or compete. *

Throughout the school year, students will attend meetings, tournaments, fundraisers and charity events that are designed to support their education, promote community service, improve skills or encourage positive behavior. Staff, parents and local media cover these events by taking photographs or video. This may include newspaper, television, websites or other media production. This also includes our school's website, regional websites, classroom and club web pages. By checking the box below, you agree that you have been notified of the possibility that you may be included in photographs or video and authorize the use for public print, display or broadcast. *

TWHS Bass Fishing Team Mission Statement *
To create a successful TWHS Bass Fishing Team, our members must adhere to the following standards: (1) Strive for excellence in the classroom as well as in tournaments. UIL regulations apply to academics. (2) Show respect to teachers and coaches by listening, following instructions, and asking questions when unclear. (3) Work hard and improve by practicing. (4) Be a positive representative of the bass fishing program at TWHS and in the community. (5) Be a positive role model for younger anglers in our program. (6) Refrain from any overt displays of displeasure or frustration during competition. (7) Take responsibility to help my teammates. (8) Uphold the policies and procedures set forth in the policies and procedures for TWHS BFT. *
Membership Fee and Tournament Fees ($150)
Membership includes team t-shirt, registration fees for all inshore tournaments including season championship for qualifying teams, membership/insurance to Student Angler Federation/The Bass Federation/FLW, all team events, tournament hospitality, awards, and prizes. *
