Family Access Membership (FAM) Application

The deadline for FAM Applications is rolling. You may apply at any time during the year. Returning FAM members must re-apply at the beginning of each season (August of each year).

1. Instructions

What do you need to prepare and submit?

1. If you are applying for financial assistance for tickets only: Complete this FAM Application only.

2. If you are applying for a scholarship for your student to attend classes and camps for the first time this 2023-2024 season (July 25th, 2023 - Aug. 18th, 2024):
Complete this FAM Application AND the Scholarship Application.

3. If you are applying for a camp or class scholarship in the Fall of 2023, or the Winter/Spring of 2024, and have previously filled out this FAM Application Form for the 2023-24 season (July 25th, 2023 - Aug. 18th, 2024) and none of your financial situations have changed: Fill out the Scholarship Application only.

Please note that incomplete FAM Applications will not be processed until complete documentation has been provided.

3. Statement of Interest

Please explain why you would like your family to be a part of Nashville Children's Theatre's Family Access Membership Program. *

Are there any other circumstances in your household or current financial issues that might further influence your admittance into the program? (This is a good place to describe things that aren't reflected on your tax statement.) *

Please list the names and ages of all immediate family members who will receive benefits from this program. *

4. Required Documents

Please upload your most recent tax document that displays your adjusted gross income and family size. This can either be in PDF or any other image format (JPEG, TIFF, etc)--please do not upload .tax or .t17 files (if scanning is not possible, a photo of this documentation is acceptable if the information needed is legible).

If you do not file taxes, documentation of income is also admissible.

Please mark out all social security security numbers before sending.
